1 · Receipt
The pharmacy delivers
Today Not built yet.
Full product Each delivery is logged drug by drug. By hand first, and later from a pharmacy-services feed.
How it works
The full product follows each controlled medicine from pharmacy delivery to final disposal. Each stage below says what the browser prototype does today and what the full product adds.
1 · Receipt

Full product Each delivery is logged drug by drug. By hand first, and later from a pharmacy-services feed.
2 · Count
Today A count that matches takes one tap per medicine.
Full product A mismatch needs a reason. How often counts are asked for follows the hospice’s own policy: every visit, Schedule II only, or higher-risk homes only.
3 · Review
Today The administrator sees two kinds of open items: a death disposal not yet documented in the chart, and a first controlled-drug order with no note that the written disposal policy was given and discussed.
Full product Every gap routes to the administrator. Corrections are added. Nothing is erased.
4 · Disposal
Today A death visit captures a witness and signatures.
Full product Witnessed disposal at death, discharge, or revocation, following the rules of the hospice’s state.
5 · Close-out
Today A death visit cannot close while medicines remain.
Full product A visit cannot close while any Schedule II through V medicine is unaccounted for.
Today The prototype produces a note for the chart the hospice already uses. For a death disposal, the note lists the drug, dosage, route, quantity, time, date, and manner. For a first order, it records that the written disposal policy was given and discussed.
MedsVouch is not the chart. It does not know whether the chart received the note.
In the prototype, the record stays on the device where it was entered. Nothing is sent to MedsVouch. Getting a connection later does not send it anywhere.
It sends nothing to the DEA, a state, or Illinois.
Planned In the full product, every event writes to one ledger, by patient, home, nurse, and time.
Planned On top of that ledger, MedsVouch will flag unusual discrepancies, waste, and timing across patients and staff. Each flag is a signal for a person to review, with the reason it was raised. It is never an accusation and never proof of diversion. This is not built yet.
Planned Outputs: an administrator’s open-items queue, a chart note ready for the EMR, an audit packet, and a pilot report in the hospice’s own numbers.
Wherever the hospice itself holds custody. Patients’ homes come first. Hospice residences and inpatient units come next. Nursing homes and assisted living are out of scope where the facility holds custody through its own systems.
